GOOD FAITH ESTIMATE

His Story is committed to working in full transparency and making sure you have informed consent for any work you do with us. It’s important that you fully understand your financial commitment to coaching and counseling. A Good Faith Estimate (GFE) is a tool, should you require it, that provides you with an understanding of your journey with us so that you can make informed decisions.

 

Helpful Information

Session fees: Session fees vary based on the coach or counselor you choose to work with or are assigned. This is the cost paid, at the time of service, for one meeting. Your cost will be defined in your consent paperwork and will only vary from that (up or down) if agreed to in writing between you and the His Story office.

Late Payment / Cancellation Fees: These are fees that may be charged by your coach or counselor if you cancel your appointment outside of the allotted grace period or fail to show up for a scheduled appointment. These fees are defined in your consent paperwork.

Frequency and Quantity of Sessions: Counseling is not defined by a set number of sessions or a desired frequency of meetings. With your coach or counselor’s input, you will have complete control of how many sessions and how often you attend.

Insurance Coverage: His Story does not file for payment through insurance but is happy to provide you with a statement of care or Superbill so that you can submit for out-of-pocket coverage. His Story is unable to verify out of network coverage or work with your insurance company in any way. We are a self-pay organization.

Third Party Billing: His Story works with third-party providers including family members, churches, and non-profits to provide full or partial payments for clients. If this is your situation, His Story will provide payment arrangements up front so you are aware of your responsibilities.

Why is a Good Faith Estimate Important?

Transparency: Trust is important. Our desire is to help you and to do that we must be transparent and forthcoming to avoid frustration and relational strife. You deserve to be accurately informed.

Informed Decision Making: There are many options in our area when it comes to coaching and counseling and you deserve accurate information to make an informed decision for you and your family.

It’s Your Right: The No Surprises Act of 2023 entitles all clients to a financial estimate of services. Visit www.cms.gov/medical-bill-rights/know-your-rights/no-insurance for more information.

How to Obtain a Good Faith Estimate: Your consent paperwork will cover all your fees. However, if you require a good faith estimate, you can contact our front office at info@his-story.org at least three days prior to your first session and request a GFE.
